Mission: The mission of PCC Community Wellness Center is to improve health outcomes for the medically underserved community through the provision of high quality, affordable, and accessible primary health care and support services. SERVICES AND SERVICE PROVIDERS
STD Testing
PCC Melrose Park Family Health Center, 1111 Superior St., Ste. 101, Melrose Park, IL 60160
This location opens at 8:30 AM on the 1st and 2nd Wednesdays of the month. This location opens at 9:30 AM on the 3rd and 4th Wednesdays of the month. Must register as a patient to receive services. There may be a cost related to registering as a new patient. Comprehensive primary care produces better health outcomes among patients and lowers health care costs. PCC offers a wide-range of primary care services to patients of all ages, and no one is denied services because of inability to pay. PCC accepts many major health plans, Mediciad, Medicare, as well as offers a sliding fee discount program for patients who do not have insurance. Services provided include: Pediatric and Adolescent health care that includes well-child exams, immunizations, sick infant/child exams, school and sports physicals, and lead screenings; Womens Health Care (ob/gyn) that includes maternity care and delivery, Pap smears, and breast cancer screenings; Adult Health Care; Senior Health Care; Diagnosis and treatment of illness; disease screening and prevention; family planning (birth control); management of chronic illnesses such as asthma, diabetes, and high blood pressure; and HIV testing and counseling. You do not need to be registered as a patient in order to set up an appointment for a free rapid HIV test; however only the PCCs Lake Street location has routine HIV testing (14 Lake St., in Oak Park, IL 60302- & the phone number for the Oak Park location is: (708) 383-0113).

Must register as a patient to receive services. There may be a cost related to registering as a new patient. This location offers limited Saturday hours; it is open only on the 2nd Saturday of the month. The location other hours are Monday through Friday. Must register as a patient to receive services. PCC Walk-In Wellness Center at Norwegian American Hospital, 1044 N. Francisco Ave., Chicago, IL 60622
PCC Walk-In Wellness Center is not a substitute for ongoing medical care with a primary care provider. It is also not a substitute for the emergency department for true emergencies. PCC Walk-In Wellness Center at Norwegian American Hospital offers non-urgent care for illnesses and injuries that are not true emergencies. Patients can be seen without a scheduled appointment for a variety of health conditions. In addition any patient that does not have a primary care provider can follow-up with a primary care provider at one of their seven health centers. PCC Walk-In Wellness Center is located just down the hall from the emergency department in Norwegian American Hospital. They provide services for: sprains, strains, and fractures; minor cuts, burns, abrasions, and lacerations; sore throats, ear aches, and sinus infections; colds, flu, and fever; back pain, joint pain, and muscle pain; abdominal pain; suspected STD infection; bronchitis and persistent cough; headaches and migraines; skin rashes and allergies; and suspected bladder or kidney infections.

Support Groups - HIV Impacted LGBTQ Adults
PCC Community Wellness Center- at Unity Temple, 124 N. Kenliwoth Ave., Oak Park, IL 60301
Latino men's group for Gay, Bi, & Transwomen. Must be at least 18 years old to paticipate.Discusses issues around sexuality and culture. Groups run weekly. Dropping into the group is fine. The group has a brief intake process. This process ensures that the participants are in the correct demographic for the group. The intake process also helps ensure that they can manage themselves fairly well in an unstructured group setting. If someone were to have mental health issues, they would be referred to another organization. This location uses space rented out from Unity Temple. Free HIV testing is also available at this site- during the specified hours- targeting Latino Gay, Bi, and Transgender individuals.
