The goal of CCHC's abstinence education program (The Advantage Plan - TAP) is to increase the number of African American youth ages 12-18 who choose to remain abstinent until marriage by providing health education which equips them to make healthy decisions and develop future and life goals. Reaching African American youth with the message of abstinence until marriage promotes a healthy lifestyle by teaching the social, psychological, and health gains to be realized by abstaining from sexual activity, and also teaches that abstinence is the only way to avoid out-of-wedlock pregnancy, sexually-transmitted diseases, and other associated health problems. Provides Housing and Shelter services, specifically for those with HIV/AIDS. Is a recipient of a Housing Opportunities for Persons with AIDS (HOWPA) grant. Our Internal Medicine Physicians focus on adult medicine and acute health issues. The focus is on wellness and disease management. Our medical director is a board certified pediatrician who is recognized for his expertise in pediatric care issues. In addition to being one of Chicagos most respected pediatricians, our Family Practice Physicians also provide quality pediatric care. These board certified physicians are trained to diagnose and treat health issues that commonly affect children. Closed on the 5th Saturday of the month (if applicable)

A trained pastoral counselor is available to address spiritual and social issues of CCHC clients. Crisis intervention and domestic violence counseling is available. We offer special prayer for anonymous requests weekly. Closed on the 5th Saturday of the month (if applicable).
