Case Management Services

WIC (Women, Infants and Children) is a federally funded supplemental food and nutrition education program intended to improve the nutritional status of eligible pregnant, postpartum, and breastfeeding women, infants, and children up to the age of five. To qualify, participants must live in Whiteside County, meet federal income guidelines, and have medical and/or nutritional risks that the WIC foods may help improve. Supplemental WIC foods may include milk, eggs, cheese, tuna, carrots, dried beans, peas, peanut butter, cereal, juice, or iron-fortified infant formula. Family Case Management (FCM) is a program designed to help women and children get off to a healthy start&and to stay healthy. At the heart of FCM is an emphasis on obtaining recommended well child examinations (check-ups), medical screenings, and immunizations.

Dental Care / Oral Health

Provides preventative and comprehensive dental treatment for children age 20 and below including dental school exams. Basic dentistry for adults only - no dentures, bridges, or braces.

Medical Health Care

Provides confidential services to all people of childbearing age, including pap exam, birth control supplies, pregnancy testing and emergency contraception. To ensure that income or lack of insurance is not a barrier to patients who are under 200% of the federal poverty level, each patients family size and income will determine their rate based on a sliding fee scale. The minimum fee per visit is $25.00. For those with Medicaid, Medicare, or private insurance, the copay amount for your plan will be due at the time of service.

Mental Health Care - Counseling

Provides support services to persons experiencing problems related to emotional and mental illness, or drug and alcohol addiction.

STD Testing

No one will be refused care based on inability to pay. The minimum fee per visit is $25.00. For those with Medicaid, Medicare, or private insurance, the copay amount for your plan will be due at the time of service.

Disease Investigation

Communicable Disease Staff investigate and track all infectious diseases that are identified in the Communicable Disease Codes of the Illinois Department of Public Health. Reports of such diseases should be reported to the health department.
