The Chicago Department of Public Health (CDPH) continues to provide mammograms, family case management and the Women, Infants and Children program at this location. Previously, CDPH provided STI and HIV services at this location. Now, however, the Erie Division Street Health Center provides these services at this location. CDPH's HIV Primary Care Program provides care for persons living with HIV. These programs provide integrated, life-sustaining services that include primary medical care, HIV treatment, mental health assistance, and comprehensive support to promote healthy living for our clients. Our caring teams of HIV specialists include physicians, nurses, psychologists, nutritionists, case managers, and medical assistants who provide comprehensive, integrated, multidisciplinary HIV specialty care services tailored to individual client needs. Services are offered by appointment. Location DOES NOT provide HIV screenings. County Care insurance is not accepted at this time.

Limited supply for individuals 19 through 26 years of age. Vaccine is available for individuals with no insurance, but if you do have insurance they will see if you qualify by using your social security number. No appointment is necessary; vaccinations are given on a first come, first served basis.
