HIV Testing

Circle Family HealthCare (CFHCN) Network provides comprehensive behavioral and primary medical care for those individuals with HIV. Integrated into our staff of physicians and care providers who address the medical management and clinical care needs of patients with HIV, our Early Intervention Program (EIP) staff provides HIV testing in outreach settings, counseling, and case management services in West and South Side Chicago communities. A unique strength of our program is that persons who are identified as being HIV positive through testing in our clinics or in the community through outreach efforts are able to immediately initiate a continuum of care with a CFHCN physician, facilitated by our outreach workers and case managers who even accommodate transportation, when needed. Circle Family HealthCare Network comprehensive services include: primary and preventive healthcare services, individual and family counseling, family support services, basic living skills, psychotherapy, case management, primary medical care and outreach services to the homeless, psychosocial rehabilitation, immunizations, lab work, dental screenings and referrals, eye exams, mental health assessments, behavior health residential program, family medicine, TB testing, early intervention, and STI/HIV/AIDS outreach and health education. Our healthcare professionals have a passion to provide specialized healthcare to residents of underserved communities. HIV primary medical care has been provided by CFHCN since the early 1980s, and our EIP program has been serving communities for 17 years. Over a decade, epidemiological HIV/AIDS data has shown that Chicagos Westside maintains some of the highest prevalence rates in Chicago (Silva, 1999). The African American and Latino communities suffer great disparities with a higher incidence of HIV and AIDS than other groups. Included in our key strategic initiatives, CFHCN is aggressively pursuing innovations within our HIV/AIDS program as well as providing greater community access for testing and case management. We seek to increase awareness and resources to better address the HIV/AIDS dilemma in the communities we serve.
