SERVICE PROVIDER INFORMATION Dental Care / Oral Health

Heartland Center for Oral Health operates three locations at which patients may receive dental care: Uptown and Englewood. Our centers provide comprehensive oral health care to patients who are homeless as well as HIV-positive. We provide a range of oral health services, including x-rays, examinations, dental cleanings, fillings, dentures, and extractions. Must be homeless and have a referral from homeless agency as proof. Must be HIV positive and have a referral from a physician. Free, except for some services (dentures, crowns, and bridges). (If homeless, must be getting services through Heartland, but if HIV, can be getting other services elsewhere.)
