Case Management Services
Heartland Alliance - Neon Street Program, 4822 N. Broadway Ave., 2nd Fl., Chicago, IL 60640
As a component of the "Home" and "Dorm" Programs, this service is offered to DCFS wards as well as homeless youth between the ages of 18 and 21 in order to provide these individuals with life skills training that includes budgeting, money management, paying bills, tenant's rights, and assistance with other activities of daily living.

Dental Care / Oral Health
Heartland Center for Oral Health - Englewood, 641 W. 63rd St., Chicago, IL 60621
Heartland Center for Oral Health operates three locations at which patients may receive dental care: Uptown and Englewood. Our centers provide comprehensive oral health care to patients who are homeless as well as HIV-positive. We provide a range of oral health services, including x-rays, examinations, dental cleanings, fillings, dentures, and extractions. Must be homeless and have a referral from homeless agency as proof. Must be HIV positive and have a referral from a physician. Free, except for some services (dentures, crowns, and bridges). Complete referral form from medical provider (proof of diagnosis).

Heartland Center for Oral Health operates three locations at which patients may receive dental care: Uptown and Englewood. Heartland Alliance - Neon Street Program, 4506 N. Sheridan St., Chicago, IL 60640
The Neon Street Home Program is for anyone that has proof of homelessnes; youth 14-21 can dropin to use day services around getting a meal, job services, using computers, etc., but must complete paperwork during an initial intake; drop-in hours are M-F 9AM-9PM; residential program is for youth ages 18-21; provides the experience of adult-living in a supervised, independent living program.

Housing

The HIV Housing Advocacy Program (HAP) prevents homelessness by providing rental/mortgage payments to a participant's landlord to help the participant avoid a housing crisis. Participants also receive ongoing support to establish and meet not only their immediate financial needs, but also their long-term housing goals and achieve housing stability. HIV positive individuals or their advocates must apply for a HAP payment through their HIV case manager. If an individual is not currently receiving HIV case management, the AIDS Foundation of Chicago will provide an HIV case management referral. The HIV case manager will assess the client's housing needs. Based on their findings, a HAP payment can be requested. Participants must be referred through the Coordinated Housing Advocacy Program of the AIDS Foundation of Chicago (AFC) by either calling AFC directly (312.922.2322) or through their AFC coordinated case manager.

The EPOCH Program is a supportive housing program for HIV positive, single individuals who currently have or have had a history of substance abuse issues. Participants are housed in scattered-site apartments mainly on the North Side of Chicago and are required to pay 30 percent of their income towards rent. EPOCH also provides case management, educational support groups, transportation assistance, and referrals to substance use treatment.

Emergency Housing

A transitional living program for homeless youth ages 18 to 21 in a dormitory setting. Youth receive case management, life skills training and supportive services, in a home-like atmosphere. An individual plan is created for each youth with emphasis on learning independent living skills. All youth are required to be in school, employed or actively searching for employment. "Practice makes progress."

Medical Health Care

Two different programs for people who are homeless and people with HIV. Health Care for the Homeless program is located in the Chicago Uptown community at Heartland Health Center - Uptown and provides primary health care and oral health care for people who are experiencing homelessness, including people who are sheltered but not in permanent housing. This program also provides primary care services at outreach sites where people experiencing homelessness stay throughout the Chicago metropolitan area. Additional services include case management, assistance with benefits and entitlements as well as referrals for mental health and/or substance use treatment or medical services not provided at the health center. The HIV Early Intervention program delivers core medical services and medical case management for participants infected with HIVto prevent the clinical progression of HIV/AIDS. The interdisciplinary team includes primary and mental health care providers, medical case managers, nurses, and outreach workers. Heartland Health Center – Lincoln Square provides affordable, high quality health care for babies, children, adults, and seniors living on the North Side of Chicago. We work to prevent illness and promote physical, mental, and social well-being by providing respectful services to all people, regardless of age, race, ethnicity, gender, language, sexual orientation, insurance status, or ability to pay. We also offer health care to immigrants and refugees and offer medical interpreters in many different languages. Saturday hours are 1st and 3rd Saturdays of month.

Heartland Health Center – Rogers Park provides affordable, high quality health care for babies, children, adults, and seniors living on the North Side of Chicago. We work to prevent illness and promote physical, mental, and social well-being by providing respectful services to all people, regardless of age, race, ethnicity, gender, language, sexual orientation, insurance status, or ability to pay. We also offer health care to immigrants and refugees and offer medical interpreters in many different languages. Saturday hours 2nd and 4th Saturday of the month. Hours are slightly modified (generally must be seen before 3:00) for new patients.

Services at this site are for people who are homeless and/or people who are HIV+; if in need of handicap access, referred to 4750 N. Sheridan location; CTA passes to get to hospitals or other locations will be provided if referrals are necessary; have doctors who are HIV specialists; if walk-in it's first come, first serve; typically a decent wait to be seen through making an appointment, but could be seen sooner if walk-in.

We provide mental health and substance abuse services, including individual and group therapy, to individuals who are HIV positive. Our goal is to improve the overall quality of life of individuals struggling with homelessness and various illnesses, including mental health and addictions issues. •Verification of HIV status •Negative TB test Must have appointments, but walk-ins are available for some groups.
