Two different programs for people who are homeless and people with HIV.
Health Care for the Homeless program is located in the Chicago Uptown community at Heartland Health Center - Uptown and provides primary health care and oral health care for people who are experiencing homelessness, including people who are sheltered but not in permanent housing. This program also provides primary care services at outreach sites where people experiencing homelessness stay throughout the Chicago metropolitan area. Additional services include case management, assistance with benefits and entitlements as well as referrals for mental health and/or substance use treatment or medical services not provided at the health center.
The HIV Early Intervention program delivers core medical services and medical case management for participants infected with HIVto prevent the clinical progression of HIV/AIDS. The interdisciplinary team includes primary and mental health care providers, medical case managers, nurses, and outreach workers. The team provides referrals for oral health.
